Immunization/Vaccine Requirements
All incoming UO and AEI students are required to: 1) complete a Tuberculosis (TB) Screening Questionnaire; and 2) either provide proof of vaccination/immunity for a variety of infectious diseases, or initiate an approved waiver process.
Required immunizations include: Measles, Mumps and Rubella (MMR); Varicella (chickenpox); Tetanus, Diphtheria and Pertussis (Tdap); and Meningococcal Quadrivalent (MCV4, MPSV4). While not required at this time, immunizations are also strongly recommended for Meningococcal B Strain, Hepatitis A and B, Human Papilloma Virus (HPV), and Seasonal Influenza (flu). The UO Health Center website also provides a more detailed list of vaccine and immunization requirements.
Please bring your immunization documentation with you to orientation. If you do not have documentation, complete the Certificate of Immunization Status form with the assistance of your health care provider, and bring the form with you to orientation. Without this information, you will NOT be able to register for your classes. For additional questions about immunization requirements contact the Immunization Compliance Coordinator by phone at 1-541-346-2747 or email immunization@uoregon.edu.
